The difference between any kind of psycosis and say neurosis, etc, is that the person has delusions and/or hulicinations. A delusion is when lets say you see a cat but ina delusion you may think it is a lion. Something is really there but is seen as something else. A hulicination is when there is nothing there at all and you see a lion. Most often this is that you hear things that are not there. For example you hear God telling you something or to do something. This is what is dangerous as they may think God tells them to kill X. It is very scary and like living in a nightmare. The best way to find out if people are having hulicination is just to ask them. They will tell a doctor what is going on in their head. They do not have this all the time, but have periodic episodes and in between can think and appear perfectly sane. There is also a parinoid personality. These people do not have delusions or hulicinations. They just think people are out to get them. Lets say the police stops them and gives them a ticket for speeding. They do not think "darn I got caught" but that the police are out to get them personally. Every time they happen to see a policeman, they may conclude that the police are following them and out to get them. They do not consider that it is coincidence that they just happened to see 3 police that day while driving around.
