In California at least, in 94 and earlier, one could choose to not have their local telephone company bill them for their long distance outside the lata. So even with the local long distance calls being there, the long distance was usually a separete bill. Now this may have nothing to do with anything except to explain the lack of writing on the top of the bill you mentioned. What is most important is that the Monarch Bay is not Dana Point. Monarch Bay was a home one of Nicole's Sister, I believe Dominique, lived in. Dana Point is a bit south. If I am not wrong, at one point the Monarch Bay house belonged to O.J. Simpson and he gave it to Nicole.
